• prior medical records
• Immigration documents
• translator (if needed)

• no photos needed

IME Labs (ages 15+)

Venipuncture performed onsite for standard IME lab tests

(please eat and drink before IME so you are not dehydrated)

X-rays (ages 11+)

X-rays are performed at Clear Medical Imaging:

Arrive at least 30 minutes before closing time
